Anal fissure is a condition in which the skin near the anal exit is torn when hard or thick stool passes through the anus. It is most likely to occur when forced to force hard, thick stools out due to constipation, but it can also occur due to strong diarrhea. It is more common in people who are prone to constipation and in relatively young women. It most often occurs in the 6 o’clock position when lying on your back, and symptoms include bleeding during defecation, pain, and persistent pain after defecation. When it becomes chronic, the same area gets cut over and over again, making the wound deep and sometimes causing small warts to form around it. Furthermore, the repeated cuts become scarred and the anus becomes narrower, which leads to a vicious cycle in which the anus becomes more prone to cuts during defecation. If the condition is diagnosed early, it can often be relieved with conservative treatment, but if it becomes chronic and conservative treatment does not improve the condition, surgery is usually considered. In order to prevent the onset and worsening of hemorrhoids, it is important to improve poor bowel habits such as constipation and diarrhea.
Cause
Hemorrhoids occur when the skin near the anal exit becomes torn due to heavy stress placed on the anus due to constipation or diarrhea. It has been reported that 64% of hemorrhoids are caused by chronic constipation, and about 5% are caused by diarrhea. Chronic constipation is also a risk factor for not only developing hemorrhoids but also making them worse. Hemorrhoids occur when defecating hard, thick stools that are difficult to defecate, and constipation progresses as the patient avoids defecating due to the pain, which in turn worsens as the anus gets injured when defecating again. If the hemorrhoid becomes chronic and the wound deepens, the anus becomes narrower and it becomes increasingly difficult to defecate, so it is important to treat the hemorrhoid as soon as you notice it.
Symptons
The main symptoms of hemorrhoids are bleeding and pain during defecation, and persistent pain after defecation. Bleeding is often only visible on the paper used to wipe the toilet after defecating, and there are not many cases where the bleeding becomes so red that the toilet bowl becomes red. The pain also causes people to unconsciously suppress their bowel movements, which progresses to constipation. As hemorrhoids become chronic, they become deeper and can form ulcers. Ulcers tend to accumulate waste such as feces, causing repeated inflammation and increasing pain levels. Repeated hemorrhoid cuts can lead to scarring and narrowing of the anus. When anal stenosis occurs, it becomes increasingly difficult for stool to pass through, which worsens the wound and repeats the vicious cycle.
・Examination/diagnosis
Diagnosis of anal fissure is made through medical history, visual inspection, and digital examination.
Treatment
Anal fissures are divided into three stages depending on the degree of progression: acute anal fissure, chronic anal fissure, and anal stenosis, and treatment is performed depending on the degree of progression. Acute anal fissures are simply lacerations and the wound is shallow, so in most cases it can be cured with medication. After regulating bowel movements, treatment is performed using ointments, suppositories, and medicines that soften the stool. If the anal fissure becomes chronic, it may not be cured with the above-mentioned drug therapy alone, and in that case, surgery (radical anal fissure treatment) may be considered. In recent years, a combination therapy of applying a gel of stem cell supernatant and injecting Clostridium botulinum toxin into the anal sphincter has been devised as a painless and non-incisional treatment. If chronic anal fissure is left untreated for many years, the anus will gradually become narrower. Once anal stenosis occurs, the narrowed anus must be surgically reconstructed at a specialized hospital
Precautions after treatmentand prevention of anal fissure
Early anal fissures that have not become chronic can be treated in a relatively short period of time with conservative treatments such as ointments, suppositories, and stool-softening medications, so it is best to seek medical attention as soon as you notice hemorrhoids. If you have abnormal bowel movements such as constipation or diarrhea, hemorrhoids are more likely to recur or get worse. Therefore, it is important to improve your eating habits and do moderate exercise to improve your bowel movements. Symptoms related to the anus and defecation may also be due to serious diseases such as colon cancer, so early colonoscopy is necessary.
