What is Roppongi Integrative Medical Care?
So-called “integrative medical care” is a therapy that combines modern Western medicine with complementary/alternative therapies and traditional medicine, etc., and exists in a wide variety of forms. The National Center for Complementary and Integrative Health in the United States defines “integrative medical care” as “a therapy that integrates conventional medicine with complementary and alternative therapies for which there is high-quality evidence of safety and efficacy”. It has also defined complimentary (complementary) and alternative therapies as “a variety of medical and health care systems, practices, and generative substances that are not generally regarded as conventional and usual medical care”. Concretely, the following classifications are provided.

In our clinic, we provide medical care to prevent various diseases to improve the function of about 37 trillion cells that make up our body, especially by using nutrients – proper diet, supplements, probiotics, intravenous infusions, and sugar control. It is called orthomolecular nutritional medicine, but has also been referred to as “nutritional therapy,” “corrective molecular medicine,” “molecular nutrition,” and “molecular integrative nutritional medicine”. Some of you may have heard of it at least once. The Ministry of Health, Labor and Welfare of our country also established a project team for integrative medicine within the Ministry in February 2010, and has been conducting a comprehensive study on the current status of “integrative medical care” and future measures to deal with the issue. The policy is “establishment and promotion of integrative medical care,” and the idea is to establish a scientific basis for integrative medical care from the perspective of prevention, including Chinese medicine, health supplements, and herbal medicine, which is by no means suspicious.
History of Integrative Medical Care
The four major civilizations – Egypt, Mesopotamia, India (Indus), and China – each had their own medicine. Unani, Ayurveda, and Chinese medicine, the so-called three major traditional medicine. These have been divided into Korean medicine in Korea and Chinese medicine in Japan. It is also believed that Unani, Ayurveda and Chinese medicine entered Europe and became the basis of modern Western medicine. It is thought that this will lead to today’s modern and cutting-edge medicine, which will probably evolve into a form of integrative medical care in the future. That is, organ therapy will eventually converge somewhere, and the future will be preventive medicine, holistic medicine that includes not only the body but also the mental, social, or spiritual health, in order to prevent illness, as is considered ideal. For example, unfortunately, modern Western medicine could not fully demonstrate its capabilities because lifelines such as electricity, water, and gas were cut off in the Great East Japan Earthquake. It has been reported that traditional medicine and complementary/alternative medical services were extremely helpful there.
The Future of Medical Care
Up to now, medicine has utilized a treatment-centered medicine, but from now on, it is important to move in the direction of disease prevention and health promotion, which is considered ideal, and from the experience of the Great East Japan Earthquake, it is important to enter the era of self-care from now on, in which people protect their own health. In other words, we believe that integrative medical care will be necessary in the future within the larger medical trend from preventive medicine to self-care.

Disease prevention and treatment from the viewpoint of intestinal flora
Linking Intestinal Flora Patterns to Disease
The types of bacteria that compose the intestinal flora and their proportions vary from individual to individual. Because of this, as a rule, each person has a unique intestinal flora. On the other hand, although unique to each individual, people from the same country, for example, are known to have similar intestinal flora compared to people from other countries when their living environment and conditions, such as climate and diet, are similar. Focusing on the fact that there are certain similarities among these intestinal flora, attempts to classify them according to the recent patterns (types) that make up the intestinal flora have been made mainly in the United States and Europe. In 2011, Arumugam et al. reported that human intestinal flora can be divided into three types. In this instance, three classified intestinal flora types are named “enterotypes”.Enterotype 1 is a group of intestinal bacteria with a high occupancy of the genus Bacteroides, enterotype 2 is a group with a high occupancy of the genus Prevotella, and enterotype 3 is a group with a high occupancy of the genus Ruminococcus, which are now called B-type, P-type and R-type after the initial letters of the bacterial genus that characterizes each type. Afterwards, Holmes et al. proposed the enterotype, which further divides type B into two types, B1 and B2, based on the occupancy rate of the genus Faecalibacterium (type B1 has a higher occupancy rate of the genus Faecalibacterium than type B2), and for the intestinal flora of Japanese people, they further divided it into five types of groups with a significantly high occupancy rate of the genus Bifidobacterium.
Anyway, it may be safe to say that the first three types are the enterotypes that are commonly seen worldwide across racial groups. Later clinical studies have revealed the characteristics of susceptibility to disease by enterotype.
Enterotype I: This type is characterized by high levels of Bacteroides bacteria and tends to be more common in ethnic groups with high animal protein and fat intake, such as Europeans, Americans, and Chinese. Some recent reports indicate that about 80% of Japanese people are of this type. Bacteroides are so-called “thin bacteria” that help prevent obesity. Bacteroides bacteria decompose dietary fiber and produce butyric acid, which improves metabolism by reducing fat incorporation into fat cells and stimulating fat burning in the muscles. Therefore, this type is said to tend to have fewer myocardial infarctions and cerebral infarctions. Enterotype II: This type is characterized by a large number of bacteria of the genus Prevotella. This bacterium has the ability to break down indigestible dietary fiber (resistant starch, non-digestible dextrin), and is said to be common among peoples with grain-based diets, such as those in South America, Southeast Asia, and Africa. Prevotella bacteria differentiate dietary fiber to produce the short-chain fatty acids acetic acid and propionic acid. This type is considered the healthiest because it reduces the rise in blood glucose levels and is less likely to cause diabetes. Enterotype III: This type is characterized by a large number of bacteria of the genus Ruminococcus. This type is common among ethnic groups with omnivorous tendencies and is comparatively common among Japanese. It is said to break down cellulose in dietary fiber, further increasing absorption of carbohydrates and fats, and making people more susceptible to obesity, myocardial infarction, and cerebral infarction.
As described above, the classification of intestinal flora into enterotypes has been found to be associated with lifestyle-related diseases. In our clinic, we are particularly committed to the prevention of lifestyle-related diseases from the viewpoint of enterotype, and we are working to prevent the onset of such diseases through intestinal activities for patients with obesity and fatty liver, which are said to be the beginning of the metabolic domino effect.
It has been reported that among obese individuals, the number of bacteria in the phylum Firmicutes is increased, while the number of bacteria in the phylum Bacteroides is decreased. Bacteria of the phylum Firmicutes break down indigestible dietary fiber to produce energy sources such as short-chain fatty acids. It is possible that an increase in the number of bacteria in the phylum Firmicutes may contribute to the development of obesity by increasing energy absorption efficiency. For example, it has actually been reported that a decrease in the proportion of the opportunistic bacterium Bacteroides tetaiotaomicron and a relative increase in the bacteria of the phylum Firmicutes can lead to obesity and diabetes. Tetaiotaomicron bacteria break down dietary fiber into short-chain fatty acids, which helps to reduce fat storage and blood sugar increases. That is, it is suggested that disruption of the intestinal microflora (called dysbiosis), represented by a decrease (simplification) of intestinal bacterial species, an abnormal increase in bacterial species that should be scarce, or a decrease in bacterial species that should be dominant, may decrease the number of species that produce short-chain fatty acids, resulting in a change in the host’s energy absorption efficiency, and may even promote fat accumulation by partially acting on the feeding center, thereby causing obesity.
Dullness, sagging, wrinkles, and blotches on the skin are also very much related to lifestyle-related diseases. Skin aging is related not only to oxidative stress (UV rays) but also to glycation stress. Particularly in obesity and diabetes, AGEs (Advanced Glycation End products: AGEs) are produced by the combination of excess sugar and proteins. This is called the “Glycation reaction (Maillard reaction). Lactic acid and short-chain fatty acids produced by butyric acid bacteria, bifidobacteria, and lactic acid bacteria suppress this Maillard reaction and act to reduce glycation stress. Furthermore, the age-related decrease in short-chain fatty acid-producing bacteria causes an increase in glycation stress. Glycation stress causes AGEs to be deposited in tissues and binds to RAGE (Receptor for AGEs) to generate inflammatory cytokines, which in turn induce tissue damage due to micro-inflammation, promoting skin aging. This glycation stress also results from excess triglycerides and LDL cholesterol (LDL-C), which can also cause dyslipidemia. Furthermore, lifestyle habits such as lack of sleep, overwork, excessive exercise, prolonged full-body tanning, and excessive alcohol consumption can lower the immune system and cause skin to age more rapidly, so these conditions should be avoided. Other known diseases resulting from glycation stress-induced alterations of proteins in the body include cataracts, age-related macular degeneration, osteoporosis, osteoarthritis (OA), Alzheimer’s disease, decreased ovarian function, aging hair, and atherosclerosis.
In this way, for the prevention and early detection of disease, it is necessary to first learn the status of the patient’s intestinal bacteria, and then determine the balance of the approximately 400 types of intestinal bacteria in order to determine the treatment. In our clinic, we suggest various biotics based on the results of intestinal flora analysis.

Among intestinal bacteria, good bacteria and opportunistic bacteria that produce short-chain fatty acids called butyric acid in particular are thought to be useful in the prevention of diabetes and obesity, and the direct intake of these butyric acid-producing bacteria is called probiotics, and the formulations include Mya-BM tablets, fine granules, and Biosly tablets. In addition, Japanese food manufacturers have begun to develop and sell supplements (synbiotics) containing not only bifidobacteria but also butyric acid bacteria (probiotics) and oligosaccharides and soluble dietary fiber (prebiotics) that increase butyric acid bacteria. And more recently, there has been a move toward supplying the body directly with beneficial metabolites (HYA, short-chain fatty acids, Equol, etc.) produced by the good bacteria. This is called post-biotics. The most significant benefit of this is that it facilitates disease management by more directly administering the metabolites needed to prevent disease. Of course, the basic principle remains the same as before, which is to increase good bacteria and improve the intestinal environment. Soluble fibers contained in burdocks, okra, and yams, polyphenols available from blueberries, and beta-glucans and oligosaccharides in barley and mushrooms have been proven to feed good bacteria and increase bifidobacteria and butyric acid bacteria. Solving Skin Problems with Intestinal Bacteria
◆Intestinal environment and skin
Generally, skin turnover is 28 days, but aging, UV exposure, and lack of sleep can delay it to about 56 days. Recently, it has been known that an increase in bad bacteria in the intestinal tract can adversely affect skin turnover. When the balance of intestinal bacteria is disturbed and bad bacteria (especially Welsh bacilli) increase, proteins and amino acids are metabolized by the bad bacteria, producing ammonia, hydrogen sulfide, indole, phenol, p-cresol, etc. These putrefactive substances absorbed from the intestinal tract pass through the blood vessels and reach the skin. When these components are secreted as sweat, they not only adversely affect body odor, but also accumulate on the skin, suppressing the normal differentiation of keratinocytes and delaying skin turnover. As a result, the damaged skin cannot be repaired, which is considered to cause dullness, dryness, and pigmentation of the skin. It is good to eliminate Welsh bacilli, phenol- and indole-producing bacteria among the bad bacteria, but Treg lymphocytes are not active without certain types of bacteria of the genus Clostridium among the bad bacteria.
This has the negative effect of making Crohn’s disease and ulcerative colitis more likely to occur, so eradication therapy must be done with caution. Another way to improve the balance of the intestinal flora is to get enough sleep. Recent studies have shown that good sleep improves vitamin and nucleic acid metabolism by intestinal bacteria and promotes skin metabolism. Also, growth hormone is secreted during sleep, but when sleep shortage occurs, the secretion of growth hormone decreases, cell division in the skin is inhibited, and repair of damaged skin is delayed. This is why people say that lack of sleep is bad for beauty. As in the old saying of good sleep and good bowel movements, good sleep is thought to improve intestinal flora and relieve constipation.
◆Vitamins and Skin
Others known to improve skin turnover are vitamin B2 (promotes lipid metabolism and skin health), vitamin B6 (improves skin conditions by breaking down proteins into amino acids and activating metabolism), biotin (involved in sugar recycling and amino acid metabolism and in skin formation), Hythiol (improves skin metabolism and suppresses melanin pigment production), vitamin A (involved in normal differentiation and proliferation of skin cells), and minerals such as vitamin D (regulates induction of skin cell differentiation) and zinc (protects skin cells from damage caused by reactive oxidants).
If these vitamins and minerals could be taken in the diet alone, there would be no problem. Since it is often difficult for busy modern people to take in the required amounts of these nutrients, we recommend taking supplements or drugs to fill in the missing portion.
◆Hormonal Balance and Skin
On the other hand, another cause of skin aging is a reduction in the amount of collagen in the subcutaneous tissue. It is caused by a decrease in the female hormone estrogen. Recently, instead of replacing female hormones, there has been a focus on Equol, a natural female hormone-like substance. Equol is produced by the metabolic action of equol-producing bacteria, a type of intestinal bacteria, on daidzein in soy isoflavones. Equol’s estrogenic, antioxidant, and vasodilating effects are said to increase the amount of collagen in the skin and bring moisture to the skin of menopausal women. However, it is known that not everyone has this Equol-producing bacterium, and only about half of Japanese women have it. Even if you have equol-producing bacteria, it is recommended that you take soy isoflavones daily, as equol only lasts 1-2 days after it is produced in the body. It is said that 10mg of Equol is required per day, which is equivalent to 2/3 of a pound of tofu or 1 pack of natto (fermented soybeans). Other factors that increase collagen levels include vitamin D, which stimulates collagen production in tissues, and vitamin C, which is involved in the production of collagen fibers in the presence of iron. Protein and iron are also important collagen ingredients, so it is recommended to eat lean meat.
NMN intravenous injection
NAD+ is a necessary coenzyme for the production of ATP (adenosine triphosphate), a source of intracellular energy. NAD+ decreases with age, and in the 50s it becomes half of what it is in the 20s. When vitamin C is administered in high concentrations, it suppresses active oxygen (free radicals) generated in tissues due to ultraviolet rays and other influences, thereby reducing inflammatory reactions through its antioxidant effect.
